ORDERING INFORMATION
To order a test at the LMM you will need the following:
- A physician (MD) to order the test (i.e. geneticist, cardiologist). A patient cannot order a genetic test for him/herself.
- You and your physician must complete the requisition and consent form for the test you would like to order (see below for test specific forms).
- Collect a blood or saliva sample to run the test (see below for additional specimen options).
- Determine how payment for the test will be made (see payment section for these options).
- Send completed requisition and consent form along with your sample to the Laboratory for Molecular Medicine (see below for shipping requirements).
- Once testing is completed, your physician will be sent the test results to discuss with you.

Specimen Requirements
For Diagnostic, Presymptomatic and Carrier Testing (except for EGFR and Prenatal Testing)::
- 7ml of whole blood (3-5ml for
an infant) in a lavender top tube (K3EDTA) OR
- 10 ug of DNA at a minimum concentration of 5 ng/ul.
Please provide DNA concentration.
- Please label sample with patient name and date of birth.
- Send in sample with completed requisition
form and signed patient consent form.
A saliva sample is acceptable for the following tests:
- Known mutation testing for all gene tests currently offered
- PTPN11 Gene Test
- In place of the blood sample required for EGFR testing (see EGFR sample requirements)
Please contact the LMM to have a saliva collection kit mailed to you.

accepts samples Monday through Friday from 9am to 5pm. Labeled samples should
be sent overnight at room temperature and accompanied by a completed
requisition form with signed consent. Please ship samples to:
